NeighborWorks Umpqua (NWU) is a rural focused community development corporation working in Southern Oregon. We work to promote opportunity for all by developing, owning, and providing access to quality housing that is affordable, increasing access to capital to build household wealth, assets and expand homeownership, improve financial resilience, and support community-based economic development efforts.

NWU was founded over 30 years ago to develop quality affordable rental housing in response to a market that was not producing housing the community needed. Over the years, we have continued to develop housing, expanding into other areas of community development in response to gaps in the market. We have an established real estate development department that has a steady pipeline of projects and continues to explore additional opportunities. As this grows, we are looking to add talented, dedicated, diverse, and forward-thinking team members.
We are searching for a Community Health Worker (CHW) who is a frontline healthcare professional and advocate who plays a crucial role in bridging the gap between health services and the community. Aligned with NeighborWorks Umpqua’s values of empowerment and inclusion, this role helps individuals and families access essential healthcare resources while promoting healthier behaviors and supporting overall well-being. As a trusted advocate, the Community Health Worker empowers community by providing education, informal counseling, and guidance on navigating complex healthcare and social service systems. By working directly with diverse populations, they help make the healthcare system more accessible, culturally responsive, equitable and patient-centered, embodying our values of Dignity, Empowerment, and Equity in every interaction.

 
As a Community Health Worker you would be :
  • Assist individuals and families in accessing and navigating health services.
  • Facilitate communication and client empowerment by helping clients effectively interact with health care and social service systems.
  • Support clients in understanding their health conditions(s), offering guidance to develop strategies for improving their health and wellbeing. 
  • Assist Oregon Health Plan (OHP) recipients with eligibility and implementation of Health Related Social Needs (HRSN) initiatives
  • Communicate health information clearly and using culturally appropriate terms and concepts.
  • Provide information, support and follow-up as needed with patients.
  • Engage in outreach activities to raise awareness of available health resources and services in the community.
  • Ensure accurate and timely invoicing for services delivered, in compliance with organizational and regulatory guidelines.
  • Maintain regular contact with patients to monitor progress, follow-up on care plans, and address ongoing needs.
  • Collect, track and report data on participant outcomes, demographics, community health trends, social determinants of health, and client needs.
  • Coordinate with healthcare professionals to ensure smooth transitions between community based care and clinical services.
  • Serve as a liaison between patients and their healthcare providers to address communication barriers and promote health literacy.
  • Ensure compliance with organizational policies, privacy standards (like HIPPA), and all relevant local, state or federal regulations.
  • Maintain accurate, up-to-date records on patient interactions, referrals, and progress.
  • Engage in ongoing training and professional development related to community health, healthcare systems, and culturally responsive care.
  • Stay informed about local, state, and federal health policies and resources to better assist the community.

  • Participate in collaborative efforts to improve health care and so care and social services accessibility, equity, and cultural responsiveness within our service region.

Minimum Qualifications (Knowledge, Skills, and Abilities)
  • Community Health Worker Certification required
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Understanding of the healthcare system and how to help patients find necessary resources.
  • A positive attitude with a growth mindset.
  • Demonstrates skill in understanding cultural differences.
  • Demonstrable communication and basic mathematical skills.
  • Strong communication and organization skills, with the ability to collaborate across diverse teams.
  • Demonstrates support of a culture of diversity, equity, inclusion, and anti-oppression
  • Attention to detail.
  • Proficiency in MS Office or similar software with ability to learn new tools.
  • Must possess a valid Oregon driver’s license and be insurable under the NWU’s insurance policy.
  • Bilingual in English/Spanish or other languages is preferred.

The employee must regularly lift and/or move up to 20 pounds. Specific vision abilities required by this position include close vision for reading, writing, and typing. The position primarily involves working indoors, with occasional work outdoors work for events or cleanup, which may involve standing for extended periods and exposure to varying temperatures. The noise level in the office environment is generally low.

Employment Type: Full-Time, In-person, 18-month grant-funded position. While the funding is guaranteed for 18 months, there is potential for future opportunities based on funding availability and program success.
