In this role, you will:

• Works with the interdisciplinary team to execute a customized plan of care for each unique patient.
• Guides the patient through the treatment process, reviewing instructions, and scheduling appointments.
• Facilitates interaction between patients and their physician and referring physician(s) as appropriate.
• Navigates the patient and family across care settings according to the established patient care plan and clinical pathways
• Utilizes effective communication to ensure patient and caregiver(s) have appropriate information, understand, and have the capacity and tools to adhere to the individualized treatment plan.
• Contributes to quality-driven performance improvement initiatives.

We require:

• Valid New York State RN License
• Basic Life Support (BLS) certification is required.
• Excellent assessment, communication, and organizational skills.
• Proficiency with an EMR (preferred)
• Minimum of two years experience in an outpatient medical office setting.
• Healthcare or Customer Service Experience
• Patient Navigation Certification (NYI will fully support our team member(s) in achieving and maintaining specialized certification.)
• Prolonged periods of sitting at a desk working on a computer, standing, bending and reaching
• Knowledge of standard precautions and the ability to utilize appropriate PPE when administering cytotoxic agents and handling bloodborne pathogens and bodily fluids
• Reliable transportation to travel to NYI locations for patient care needs

Salary: Starting at $59 per hour based on experience
