• Manager Interviews
• General Information: Bangor Office Census averages between 250 and 275 patients
• Will you accept a traveler with 1-year experience? Yes
• Will you accept a first-time traveler? Only if they have Home Health Experience
• Patient Types/Common Diagnoses: S/P orthopedic surgeries, Wound care, Chronic/Acute CHF and COPD, s/p cardiac surgeries, genitourinary, IV antibiotic teaching/central line care, med pre-fills, lab draws
• Patient Ratios: Case Manager averages 25 to 30 patients
• Skills required: Nursing skill set to care for the common diagnosis listed above
• Support within the Department: Scheduling, Clinical Manager, Lead Support Team
• Technology/ Equipment: EMR: Netsmart, IV pumps: Provided through vendor
• Floating: If Bangor Territory Census is low may ask traveler to support other agency locations, if census allows.
• Orientation: 1 week

• Driver’s License? Yes, driver’s license and must use own vehicle to drive from patient home to patient home.

Schedule Information

• Weekend rotation: Typically once a month, could be more dependent upon staffing
• Call requirement: Overnight - 5 PM to 8 AM. Rotation is typically twice a month but could be more dependent upon staffing
• Holiday Expectations: Will be required if assignment aligns with major holiday (New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas)
• RTO: Manager approval
• Shift times: 5 (8) hour days, Monday thru Friday, weekend and on call rotation, business hours 8 AM to 5 PM
