As a Case Manager (CM) Certified , you will play a vital role in planning and managing care for our targeted patient population. Collaborating with physicians and an interprofessional healthcare team, you will ensure that our patients receive compassionate, efficient, and high-quality care.

Your Impactful Responsibilities

In this dynamic role, you will:

  • Collaborate with healthcare professionals to monitor patient progress and ensure care plans are patient-focused and effective.
  • Educate staff and students, serving as a preceptor and resource for case management topics.
  • Drive Improvement by identifying areas for performance enhancement and fostering a culture of peer accountability.
  • Review Medical Necessity for admissions and continued stays, ensuring compliance with guidelines.
  • Facilitate Discharge Planning by anticipating needs and managing emergent situations to prevent readmissions.
  • Document assessments and interventions efficiently to promote continuous improvement.
  • Monitor Financial Metrics to contribute to departmental and hospital financial targets.
  • Innovate by identifying opportunities for practice changes and engaging in performance improvement projects.

What We’re Looking For

To thrive in this role, you should have:

  • Education: A Bachelor’s degree in nursing (Master’s preferred).
  • Experience: Five years of clinical nursing experience, including two years in case management.
  • Licenses & Certifications:
    • Registered Nurse - Texas State Licensure (or Compact State Licensure) within 60 days.
    • ANCC Recognized Certification in Case Management or Accredited Case Manager certification.
  • Skills: Strong communication, critical thinking, and organizational skills, along with proficiency in Microsoft Office Suite.

Working Conditions

As a Case Manager, you will:

  • Dress in business professional attire .
  • Be available for on-call duties during emergencies.
  • May require travel within the Houston Metropolitan area .
