CHCFC is seeking a dedicated and compassionate Community Health Worker to join our team! This role is essential in providing culturally competent health education and support services to underserved communities. The ideal candidate will have a strong commitment to improving health outcomes and a passion for working with diverse populations, and will be a Certified Community Health Worker or be willing to obtain certification within one year of hire.

• Conduct outreach activities to identify and engage individuals in need of health services.
• Provide health education on various topics, including chronic disease management, preventive care, and healthy lifestyle choices.
• Assist patients in navigating the healthcare system, including scheduling appointments, accessing resources, and understanding insurance coverage.
• Facilitate health screenings and assessments, and refer patients to appropriate healthcare providers.
• Collaborate with community partners to address social determinants of health, such as housing, food insecurity, and transportation.
• Maintain accurate and up-to-date patient records.
• Participate in team meetings and trainings to enhance professional development.

• High school diploma or equivalent required. Community Health Worker Certification perferred.
• Experience working in community health or social services preferred.
• Strong interpersonal and communication skills required.
• Ability to work independently and as part of a team.
• Bilingual skills (English and Spanish) a plus.
• Valid driver's license and reliable transportation required.
